Eligible Participants must submit all required documentation to qualify for COVID-19 assistance.

Eligible Participants​
-
Must rent a home or apartment within the corporate City Limits of the City of Sapulpa in Creek and Tulsa Counties that is participant's primary residence.

Must be at least two months behind in rent.

Must have a written notice from landlord indicating delinquency in rent payment.

Must have applicant's photo ID, lease in applicant's name, and show responsibility for paying rent.

Must have one or more adult members of the household who has experienced a documented, involuntary loss of income that occurred due to the COVID-19 public health emergency.

Must show the date of job loss that occurred after March 13, 2020.

Must be able to show documented annual income at the time incident and documented loss of income due to COVID-19.
